Athletic Clearance
In order to participate in Millikan Athletics, a Parent/Guardian and student must complete the online clearance process. Sports Clearance forms for the 2025-2026 school year are due to the Nurse on the dates below in order to be cleared when the season starts.
Pre-Season Fall: July 7, 2025
Fall: August 4, 2025
Winter: October 17, 2025
Spring: January 16, 2026
Due to the high volume of Sports clearances, we are unable to process last minute requests.
Athletic Clearance During the School Year:
To begin the sports clearance process for the 25-26 school year please follow the steps below:
- Create an account on www.AthleticClearance.com Athletic Clearance Guide
- Print out the Confirmation Page, please be sure to sign and date
- Attach the completed Physical Exam form to the Confirmation page
- Submit the Confirmation Page and Physical Exam form to the School Nurse
Medical Insurance Required
In order to participate in athletics, all student-athletes must have a valid medical insurance coverage. If you do not have current medical insurance, you may purchase a plan through Myers-Steven-Toohey Insurance.
